The American Board of Craniofacial Dental Sleep Medicine ("ABCDSM," "we," "us") is an Illinois not-for-profit corporation that credentials dentists in craniofacial dental sleep medicine. This policy explains what information we collect from candidates, Diplomates, and visitors to abcdsm.org, how we use it, and the limits we place on sharing it.
Our commitment is simple: we do not sell, rent, trade, or give away your contact information. Your name and credential appear on our public provider map. Your email address, phone number, and mailing address do not leave ABCDSM except as described below.
1. Information we collect
We collect information you give us when you apply for credentialing, renew, register for continuing education, take an examination, make a payment, or contact us. This may include:
- Name, professional degree, and credential designations
- Practice name, practice address, email address, and phone number
- Dental license information and verification
- Continuing education records and certificates
- Examination registration, results, and proctoring records
- Case summaries and case studies submitted for credentialing
- Billing details needed to process dues and fees
Our website uses cookies to analyze traffic and improve the site. This information is collected in aggregate form, such as pages viewed, browser type, and the site you came from, and does not identify you personally. You do not have to give us any personal information to browse our website.
2. Patient information in case submissions
Case summaries and case studies must be de-identified before they are submitted. Please remove names, dates of birth, addresses, record numbers, full-face photographs, and any other details that could identify a patient. ABCDSM does not request, and does not want to receive, protected health information. If identifiable patient information is submitted by mistake, we will return or securely delete it and ask for a de-identified version.
3. How we use your information
- To review applications and award, renew, and verify credentials
- To track continuing education requirements
- To administer and proctor examinations
- To process dues, fees, and course registrations
- To send you Board news, renewal reminders, and educational opportunities
- To keep our records accurate and meet legal and tax obligations
4. Our public provider map
To help patients and colleagues find credentialed providers, ABCDSM publishes a provider map of Diplomates in good standing. Each listing shows only your name, credential designation, and practice city and state. We may also confirm whether a person currently holds ABCDSM credentials when asked.
You may ask to be removed from the provider map at any time by writing to admin@abcdsm.org. Removing your listing does not affect your credential.
5. When we share information
We share personal information only in these situations:
- Provider map information. The details listed in Section 4 are public and may be viewed or referenced by anyone, including other professional organizations.
- Service providers. We use outside services to run our website and online store, learning platform, exam proctoring, payments, and email. They receive only what they need to do that work for us and may not use it for their own purposes.
- With your consent. If another organization asks for your contact information, we will not provide it unless you have told us in writing that you agree.
- Legal requirements. We may disclose information when required by law, subpoena, or court order, or to protect the rights and safety of ABCDSM and its members.
We do not share email addresses, phone numbers, or mailing addresses with other organizations, sponsors, or vendors for their own outreach or marketing.
6. How we protect your information
Access to member records is limited to the Board and the staff who need it to do their work. We use reputable service providers with their own security safeguards. No system is perfectly secure, but we take reasonable steps to protect your information and will notify affected members as required by law if a breach occurs.
7. How long we keep it
We keep credentialing, examination, and continuing education records for as long as needed to verify credentials and meet our legal, tax, and accreditation obligations. When records are no longer needed, we delete them or keep them in secure storage.
8. Your choices
- Ask to see the information we hold about you
- Ask us to correct anything that is inaccurate
- Ask to be removed from the provider map
- Unsubscribe from non-essential email at any time (we will still send messages needed to maintain your credential)

9. Children
Our website and services are intended for dental professionals. We do not knowingly collect information from anyone under 18.
10. Links to other sites
Our website links to other web resources. This policy does not apply to those sites, and once you follow a link you are subject to that site's privacy policy.
11. Changes to this policy
If we change this policy, we will post the new version on this page with a new date. If a change affects how we share member information, we will notify members by email before it takes effect.
